Registration will be processed through Purdue University except for complimentary registrations for conference sponsors*. All checks should be made payable to Purdue University. Purdue is also able to process credit cards (MasterCard, Visa, Discover, American Express). Registrations can be submitted online or via the registration form found on the Purdue University Conferences website (should be up by mid-January). If using the registration form, one should be submitted for each person attending the conference (both residents and preceptors). Hotel registration is a separate process and should be done by contacting the hotel directly. (See resident's checklist or preceptor's checklist) Pre-conference Preceptor Workshop also has a separate registration form. The registration fee is $100 (if paid by March 1) and must be paid for each attendee (preceptors and residents). Registrations not postmarked by March 1 will be accepted with a $200 late registration fee. Although there are no refunds for conference registrations fees, registrations are transferable between attendees. * Complimentary registrations should be faxed to 317-962-2991 clearly marked "complimentary registration" and your affiliated company.
