Statement of the purpose

Statement of methods used

bullet

Summary of (preliminary) results to support conclusion

bullet

Conclusions reached

bulletThe Abstract must include two learning objectives and two self-assessment questions.
bullet

All residents presenting an abstract must submit a curriculum vitae (CV) with their abstract.  This is essential for Continuing Education credit.

bullet Abstracts must be completed ONLINE no later than February 1, 2009 at 11:59:59 PM - please use Internet Explorer versions greater than 5.0
bulletRemember that conference registration must also be submitted. 

ABSTRACT SUBMISSION INSTRUCTIONS:

A complete mailing address is required.  

bullet

Short specific titles are desirable. 

bullet

Authors should be listed with the first name first, middle initial, and the last name last; do not list title and degrees. Place an asterisk by the presenter's name.

bullet

No tables, graphs, multiple column text may be included in the abstract.

bullet

Do not indent or justify paragraphs. Do not use carriage returns except between paragraphs (i.e. leave all line wrap decisions to the word processing program). Do not manually hyphenate words at the end of lines (unless the word is always hyphenated).

Check spelling and punctuation carefully.  Abstracts will be printed in the abstract book exactly as submitted.

Total word count for the body of the abstract is 300 words.  This does NOT include title, authors, learning objectives, or self-assessment questions.

For a successful submission of an abstract, it is recommended that you prepare your abstract in a word processing document, have your preceptors review it, and then cut and paste into the online submission form.

Disclosure and Eliminating Bias.  Your presentation should be free of any remarks that would show bias.  Also, affiliations of you and your spouse must be disclosed to us on the Faculty Disclosure as you submit your abstract online.  Any affiliations will be disclosed before finalizing the program to allow the committee to resolve your conflict.  It is possible the committee will determine it is not appropriate for you to continue.  You must also place the disclosure information in front of your slides.  If you have no information to disclose you must specifically state that as well.

You will receive a confirmation number after a successful submission.

bullet

Failure to comply with abstract requirements may result in an abstract being rejected.
